CP charges can cover a multitude of sins. At the worst, it could be videos of child rape and murder. But, at the less serious end it could be a slightly too naked child from an innocent holiday photograph, depending on the jurisdiction. At that end whether the possessor gets charged can depend on how much the law enforcement don't like them.

A lot of legitimate, adult porn sites have a few uploaders who put up illegal shit. Then legitimate viewers are looking for adult porn but through no fault of theirs they get a thumbnail ad which they ignore (might not even see) and its in the cache on their hard drive and if they get busted, well, they're fucked.

It's likely millions of innocent people who use pornhub or whatever will have passed a few underage pictures without noticing.

Yes, but it can be hard to do depending on the device. I've heard of cases in the past where the FBI said they found CP or hacking materials but the forensics said the CP arrived after the device was seized :O. Windows is probably hardest because NTFS is a journaling file system. Also, modern forensic chain of evidence has got better, so disks get seized, imaged then locked up.

For example, it was there that confidential information from the health insurance company DC Health Link was published, including personal data of members of the US House of Representatives and Senate.

During the existence of BreachForums, many data leaks from a variety of companies were posted there. According to the US Department of Justice, at the time of closure, BreachForums had more than 340 thousand active users.
